How to Claim Your Gift With Purchase:
1. To receive your Gift with Purchase, purchase a qualifying Mitsubishi HDTV from an authorized Mitsubishi Retailer from
June 6, 2010 through June 19, 2010.
2. Return this completed form, including the serial number, along with a copy of your receipt with the purchase price circled.
3. Only handwritten responses will be accepted. No stickers or reprinted address labels. Product must be new and un-used.
Offer valid for purchases made from June 6, 2010 through June 19, 2010.
4. You may also register for the gift with purchase online at www.mitsubishi-tv.com/golfcluboffer2010. You are still required to mail in the
copy of your cash register receipt. Wait three to four weeks and then you can check the status of your gift with purchase online.
Terms and Conditions
1. This offer is only available to the original consumer end user purchaser. No claims from re-sellers, dealers/distributors, their employees or
warehouse facilities, groups, clubs or organizations will be accepted or acknowledged. No P.O. Boxes accepted except military APO/FPO
(Air/Army Post Office or Fleet Post Office) addresses.
2. Limit one gift with purchase per person, address or household; may not be combined with any other offer or rebate. Submissions of multiple
requests for the same individual or family may constitute fraud and may result in prosecution.

5. Claim forms must be postmarked by July 21, 2010 and received by August 21, 2010.
