How to Claim Your Gift With Purchase:
1. To receive your Gift with Purchase, purchase a qualifying Mitsubishi HDTV from an authorized Mitsubishi Retailer from
May 29, 2009 through June 21, 2009.
2. Return this completed form, including the serial number, along with a copy of your receipt with the purchase price circled.
3. Only handwritten responses will be accepted. No stickers or reprinted address labels. Product must be new and un-used.
Offer valid for purchases made from May 29, 2009 through June 21, 2009.
4. You may also register for the gift with purchase online at www.mitsubishi-tv.com/golfcluboffer2009. You are still required to mail in the copy of your cash register receipt. Wait three to four weeks and then you can check the status of your gift with purchase online.
